If I am awarded a Southeast Iowa Builders Association Construction Scholarship, and do not complete the first semester of study, I agree to refund the scholarship money (extenuating circumstances may be reviewed by the Scholarship Committee).

Additionally, I hereby authorize the Southeast Iowa Builders Association to use my name, photograph, and any information submitted to them by me in any news releases, publications, or their web site.
